- Data about the manufacturer (address, phone, fax)
- Data about the applicant (address, phone, fax)
- Certificate of registration of an applicant's company (representation).
- Requisites of firm of the applicant (representation).
- The exact name of the certified products, including types and models.
- Product or composition of products
- Technical documentation or description
- Earlier the received certificates or test reports (desirable)
